There are already "reality engines" of various sizes specified in the mechanics. I believe they do require a Demesne underneath or a Manse channeling power through a Hearthstone.

As we've discussed, expansion of Creation is a bureaucratic task proportionate to the return. One Sidereal can stretch Creation by a few miles in one spot. To extend all the border of Creation by hundreds of miles, and thus generate millions of square miles, is a task requiring hundreds of thousands of people acting in concert.
